The goal of this project to develop a healthcare organization-specific system that leverages technology and the power of transparency to identify care partners, improve patient/care partner clinician communication, and connect care partners with needed services.
Project description
In partnership with ARCHANGELS, Beth Israel Lahey Health Primary Care and two other clinical sites, Catholic Health in Buffalo and Michigan Medicine, we will develop and implement a care partner version of OurNotes that is fully integrated into the EMR/clinical notes. In addition, we will identify caregivers before and during clinical visits at 2 healthcare organizations using the Caregiver Intensity Index developed by CII. Finally, we will evaluate the effect of these interventions on patients, care partners, clinicians, and staff.

Partner Organization Details
Beth Israel Lahey Health Primary Care will serve as one of the clinical sites and be the primary site for developing the fully integrated OurNotes (led by Matt Germak and Nancy Cibotti)
ARCHANGELS will bring their considerable expertise in identifying and support care partners to the project, led by Alex Drane and Sarah Stephens Winnay.
Catholic Health, Buffalo and Michigan Medicine are also partnering with us in piloting this initiative at selected primary care clinics.
